    Download, print, complete and sign the Authorization for release of Health Information form and return them via fax or mail.

    Is there a charge for copies?

    Yes. We charge $0.25 per page. We can let you know by telephone what the total cost is to provide you with the requested records after we have an opportunity to retrieve your medical records.

    May my spouse request and receive copies of my medical record?

    No. Another person may not receive copies of your medical record without an authorization form signed by you authorizing the Medical Center to release copies to him or her.

    What are your hours of operation?

    We are open 8 a.m. to 4:30 p.m., Monday through Friday and closed on national holidays.

    When may I expect to receive my copy?

    Because of the large number of requests we receive daily, it may take up to 10 working days to fulfill your request.

    Will you fax copies to my home office?

    We normally do not fax medical information due to the confidential nature of the information.

    Will you mail the copies or will I have to pick them up?

    We can mail the copies to the address you specify on the authorization form, or you may pick them up during our hours of operation.

    How long do you keep medical records?

    Medical records of adult patients are kept for ten (10) years after the last date of service.  The records of patients who were 18 years old or less at the time of treatment are kept for one (1) year after reaching age 18 but no less than ten (10) years.

    Can medical records explain the charge on my bill?

    Unfortunately we are not able to help explain any charges on your hospital bill. Please contact Customer Service in our Finance Department at 714.456.7328.

    May I obtain a copy of my X-ray films from you?

    X-ray films are stored in the Radiology Department and we do not have access to those. Please contact Radiology’s Film library at 714.456.6281.

    How do I request a copy of my birth certificate?

    Birth Certificate copies are available through the county.  You may request a copy from the Orange County Clerk-Recorder office.
