Thank you for choosing UC Irvine Healthcare for your medical care. The following tips will help you make the most of your physician visit. Please check with your physician's office prior to your appointment for additional instructions or preparations required for your appointment.
Before Your Appointment
- Confirm your appointment. Before your visit, you may receive a recorded message confirming your appointment. If you don’t receive a call or need to reschedule, please call your physician’s office to confirm your appointment or to reschedule.
- Check your health insurance coverage. Contact your health insurance provider to verify the specifics of your coverage. Call Physician Referral at 877.UCI.DOCS (877.824.3627) if you have any managed care referral questions.
- Prepare a list of your current medications. Be prepared to discuss your prescribed medications, including how much you take and how often you take it. If you have copies of your prescriptions, bring those with you.
- Make a list of all your health concerns and issues.

Day of Your Appointment
- Be on-time for your appointment. For maps and parking information »
- Bring your list of current medications.
- Be prepared to pay any insurance copayments and/or deductibles.
- Bring the following information with you to help you fill out any necessary paperwork:
- Insurance card, including your policy number.
- Your employer's name, address and phone number (if you are insured through your employer)
- Managed care referral authorization - this is necessary if you are covered by a managed care plan in which a physician referral is required
- Name, address and phone number of your referring or primary care physician.
- Any forms you were asked to complete prior to the appointment
- Medical information requested by UC Irvine Healthcare, such as radiology results and lab diagnostic test results.
